Forms is where every questionnaire, consent form and checklist your hospital builds lives. One form is built once and used by whichever department needs it — an outpatient screening questionnaire, a theatre consent, a pre-admission history — and answered from a link, by someone who needs no account.

Where it sits

On Enterprise, Forms is inside the Records care area, beside Send and track documents. Turn the Records care area off and the whole group goes with it. Solo and team practices list Forms flat in the sidebar instead, which is why the click path in the shared guides below is one step shorter than yours. Three tabs sit under the heading:
Forms and documents sent for signature are not the same thing, and the difference matters. A form is answered by anyone who has the link and does not identify itself; a document is sent to one named patient for their signature and files itself to their record. Use a document when you need a signature attached to a person. Use a form when you need answers from many people.

Building and sharing one

The builder, the settings and the sharing link behave identically in every edition, so those guides are shared rather than repeated here. Only the click path above differs.

Who can do what

Reaching the page is not permission to act. A staff member sees Forms if their role may open it, but building, publishing and reading answers are separate permissions your owner or manager grants in :

Check it worked

Open Forms from the sidebar under Records. You should see the Forms and eSignatures tabs, a New form button, and — under Recent forms — a Search forms box beside the filters. If Records is not in your sidebar at all, the care area is switched off for your hospital — an owner or manager turns it on in Settings → Facility — see Care areas.

If something goes wrong

On Enterprise it lives inside the Records care area. If that care area is off, nothing inside it appears. An owner or manager turns it on in Settings → Facility — see Care areas.
You have View forms but not Create and edit forms. Your owner or manager grants the rest in .
Reading answers is its own permission — View form responses — and every response opened is recorded in the hospital’s audit log.
That is a document, not a form. Send and track documents covers it.
