One sheet records the whole visit. You do not queue her through triage first and you do not write the findings twice — the vitals and the obstetric findings go onto her chart together, and the contact is counted against her schedule in the same action.

Open the sheet

Select Record contact on her row in Today’s Clinic. The sheet is headed Antenatal contact and names her underneath, with her gestation beside it — check that line before you type anything.

Set the Contact date

It starts on today. You can back-date a visit that was written on paper, but not forward-date one: a date in the future is refused, because it would advance her schedule off a visit that has not happened.

Enter the vitals you took

Blood pressure · Pulse · Temperature · Respiratory rate · SpO₂ · Weight · Height.Blood pressure is entered as two numbers with a slash — the field shows 120/80 and rejects anything else with Use the form 120/80.

Enter the obstetric findings

Fundal height · Fetal heart rate · Presentation · Lie · Fetal movements · Urine protein · Urine glucose · Oedema · Pallor.The pickers all start on Not recorded, and that is a real answer — leave one there and nothing is claimed about it. The urine fields read as they do at the bench: Negative, Trace, 1+ through 4+. Fundal height carries its own note: “Read for dates on the chart, where the gestation is known.”

List any danger signs

Danger signs is a free-text line, and the field says how to use it: “Separate with commas. Shown on the chart; never acted on automatically.” Write what she reported — headache, blurred vision, reduced movements — as you would say it.

Say whether she needs a clinician today

Refer to a clinician today is a switch: “Puts her on a department’s consultation list to be seen today. Without this the findings go to her chart only.” Switch it on and two things appear — a Reason for referral box, which is required, and Send her to.

Choose where she is going

Send her to lists your departments, and the sub-units inside whichever you pick. This is what puts her in front of a doctor: a referral with no department appears on no consultation list at all, so the sheet refuses to record one — “Choose a department. Without one this referral reaches no consultation list.”On a clinic that charges for consultations, only departments she has paid for can be chosen; the rest are marked Not paid, and a consultation fee that has expired is not a valid route. A free clinic fills in its general outpatient department for you.

Select Record contact

The button reads Recording… while it saves, then the sheet closes and confirms Contact 4 recorded — with “The findings are on her chart”, or the department she is now on the consultation list for.
Everything except the date is optional, on purpose. A midwife who did not measure the fundal height must still be able to record the contact she did have. An empty box stays empty on the record — it never becomes a zero, and a zero here would look exactly like a real measurement.

Check it worked

  • The confirmation names the contact’s number — Contact 4 recorded.
  • Her row on Today’s Clinic moves from 3 of 8 to 4 of 8, and Next contact jumps to the following window.
  • Her Status badge leaves Due or Overdue.
  • On her chart, the visit is on the Vitals tab under Over time, with the obstetric findings alongside the vitals — see The pregnancy on the chart.
  • On her antenatal card it is the last row of the series, badged Latest.

Order the booking investigations

At the booking visit the same permission lets you order the antenatal profile in one action instead of six. Open her antenatal card and select Order antenatal profile. The sheet is headed Antenatal profile and names her underneath. It lists what your clinic’s laboratory catalogue can supply, each with the name it will be ordered under, all ticked: Untick anything you do not want, then select Order 6 investigations. They arrive on the laboratory worklist as pending, exactly as they would if you had ordered them one at a time.
It tells you what it cannot order, before and after. Anything in the profile that your clinic has no matching test for is listed under Not in your laboratory catalogue, by name, on the sheet — and again on the confirmation under Not ordered. The confirmation never says “profile ordered”; it says how many were ordered and which were not, because a profile that quietly ordered four of six would leave you believing the booking bloods were done. Add the missing tests under Laboratory → Tests and order the profile again, or arrange them outside the clinic.
Blood group, rhesus and genotype are recorded, not ordered. They are fields on her record, not laboratory orders — blood group and rhesus on the pregnancy record when it is opened, genotype on her patient record. The sheet says so under Recorded, not ordered so nothing looks forgotten.
Anything already ordered for this pregnancy shows Already ordered with the date and starts unticked, so pressing this twice does not draw a second specimen. You can tick it back on — a repeat blood count later in the pregnancy is a real order.

If something goes wrong

Set Contact date to today or earlier. Recording a visit before it happens would move her schedule on for a contact nobody made.
Blood pressure takes systolic, a slash, then diastolic — two or three digits each. If you did not take it, leave the field empty.
Refer to a clinician today is on and Reason for referral is blank. Write the reason, or switch the referral off.
Somebody closed the record — as delivered, or otherwise ended — while the sheet was open. Close the sheet and reload the board; if the record was closed in error, a new one has to be opened for her.
An owner grants Record an antenatal contact in the permissions sheet, under Maternity. See Permissions. The same permission covers Order antenatal profile.
Your clinic has no test matching any of the ones you ticked, so nothing was created — you have not half-ordered the profile. Add them under Laboratory → Tests, then select Order antenatal profile again.
The record was closed — as delivered, or otherwise ended. Order antenatal profile does not appear on a closed record; if you see this, somebody closed it while your sheet was open. Close the sheet and reload her card.
One message covers both, deliberately. Reload Today’s Clinic and try from a fresh row; if the row is still there and this repeats, contact support.

FAQ

No. The contact records the encounter itself — the vitals you take here are the vitals on her chart. Recording it twice would put the same visit on her record twice.
Not from this sheet. Record the correction as what it is and tell the clinician who will see her; the visit already saved stays on the record.
On her antenatal card — select her name on Today’s Clinic. It lists every contact oldest first, with the gestation she was at each.
It records that she needs to be seen and why, and it puts the visit on the consultation list of the department you chose under Send her to — which is how a doctor finds her. It does not book an appointment or send a message.Without a department she would reach no list, so the sheet will not record the referral until you pick one.The referral stays open on her antenatal card, marked Not yet seen, until somebody records that a clinician saw her. See Close a referral.
Not on this sheet. It records what you found without colouring or ranking it. Her antenatal card does name flags where the latest findings meet a threshold, and states beside them that they are shown for information and do not replace your assessment. Nothing is acted on automatically anywhere.
Not per clinic. The profile is the booking set — HIV, hepatitis B, syphilis, a blood count, urinalysis and a malaria parasite screen — and the sheet lets you untick anything you do not want before ordering. What it will not do is claim to have ordered a test your catalogue does not hold.
Because you are ordering it from an open pregnancy record, so the pregnancy is already established. Ordering a confirmatory test at that point would have you waiting on a result you do not need.
Only from a row on Today’s Clinic, so she needs an open pregnancy record first — see Open, re-date and close a pregnancy record. That is what makes the contact countable against a schedule.
