When you record an antenatal or postnatal contact you can switch on Refer to a clinician and say why. That marks the visit as needing a clinician — and it stays marked until somebody records that one saw her. This page is how that gets recorded. Until it is, the referral reads Not yet seen wherever it appears.
A referral list that is never cleared stops being useful. After a few weeks every woman on it looks the same, and the one who genuinely has not been seen is impossible to pick out. Closing referrals is what keeps the list worth reading.

On her antenatal card, the contact you referred shows Referred to a clinician with the reason. Directly under it, an open referral offers Which clinician saw her?

Choose the clinician

Pick from the staff list. Your own name is marked (you) and is selected for you if you are on the list — you can change it.

Read the sentence under the picker

It tells you which of two records you are about to make, and it changes as you change the selection:
  • Choosing yourself — “This is recorded as your own sign-off — that you saw her.”
  • Choosing somebody else — “This is recorded as your account of who saw her, not as their sign-off.”

Press Record the review

The button reads Recording… while it saves, then confirms either Recorded in your name as the clinician who saw her or Recorded as your account of which clinician saw her.

The two kinds of closure, and why they read differently

A clinician’s name on a clinical record is normally read as their signature. That is only true when the clinician put it there. So the record keeps the two apart, and every screen that shows the name shows which one it is: You never choose which of the two it is. It follows from who you picked: choose yourself and it is a sign-off, choose anyone else and it is your account of them.
Review recorded is the time the closure was written down, not the time the clinician saw her. On a reported closure those can be hours apart, which is why the label does not say “Seen at”.

Who can do it

Signing off a referral in your own name needs the Sign off a referral as the clinician who saw her permission, in . Recording which clinician saw her does not need it. Anyone who can record the contact can record that — so a referral can always be closed, even where the clinician does not use ClinikEHR.

Check it worked

  • The Not yet seen line is replaced by who saw her and on what basis.
  • On the antenatal card the referral line turns from amber to green.
  • The control disappears — a referral is closed once, and reopening one is not an act this screen offers.

Common issues

Somebody closed it while your screen was open. Reload and you will see their entry.
You are on a contact where Refer to a clinician was never switched on. A review is only recorded against a referral.
The contact was retracted — see Retract a record. A contact that says it never happened cannot carry a review.
The person you picked is no longer on this clinic’s staff. Pick somebody who is.
The staff list is empty because no staff member has a first and last name recorded. Add them in .
A temporary problem reading the staff list. Reload the page. If it keeps happening, the referral is still safely recorded as open — nothing is lost.
There is no undo on this screen. Correct it the way every other mistake in a maternity record is corrected — see Retract a record for what can and cannot be changed after the fact.
No. It records who saw her and on what basis. It books nothing, sends nothing and changes no other part of the record.
