Choose the person first. Opening the AI Note Taker with nobody selected stops you at “No client selected — Please select a client in the ‘Quick Note’ section before using the AI Note Taker.” Pick them in the Quick Note card on the Notes dashboard, then open the sheet.
Generate the note

Open the AI Note Taker
The sheet is headed AI Note Taker — “Generate structured clinical notes from recordings or documents”. It walks you through in order.

Select a template
“Select a Template — Choose the note format for your documentation.” This decides the shape of what comes back, so pick the format you would have written by hand.

Under “Customize Output”:

Record, or upload a file
Live Recording — “Speak naturally — all participants will be captured”. You can pause, resume, stop and re-record.Upload File — MP3, MP4, PDF, DOCX, TXT or MD, up to 100 MB. Use this for a recording made elsewhere, or a referral letter you want summarised into the note.

Wait for processing, then review
The session processes, then shows you the drafted note. Read it against what actually happened before you go any further.

Apply it to the note form
Applying the draft drops the text into the note form, where you edit, add what the recording missed, and sign as normal — see Write a clinical note.
Manage your AI sessions
AI Notes in the left rail opens the manager sheet over the page. Filter by Completed, Processing, Failed or Incomplete, and act on each session:
A session that has stopped making progress shows an amber Stalled badge — stop it and record again rather than waiting.
In a video consultation
Telehealth has its own note taker built into the call, which can start automatically when you begin recording. That one needs the Team or Enterprise plan. See Run a video consultation.Check it worked
The session shows as Completed in the AI Notes manager, the drafted text is in your note form under the template you chose, and the note is attached to the right person. Nothing reaches the chart until you save or sign it.If something goes wrong

The AI Note Taker is an add-on rather than a plan feature. Team and Enterprise clinic owners get it bundled; every other seat needs its own. Select Manage Add-ons in the prompt, or see Plans and add-ons.

Select the person in the Quick Note card before opening the sheet. The note taker files its output against a chart, so it needs to know whose.
The session is stuck on Processing
The session is stuck on Processing
Look for the amber Stalled badge in the AI Notes manager. If it is there, select Stop and run it again — a long recording on a weak connection is the usual cause.
My upload was rejected
My upload was rejected
Only MP3, MP4, PDF, DOCX, TXT and MD are accepted, up to 100 MB. A long recording in a lossless format will be over the limit — export it as MP3.
The note came back in the wrong format
The note came back in the wrong format
Regenerate it from the AI Notes manager against a different template. The recording is reused, so you do not have to see the patient again.
The recording missed a second speaker

Set Speaker Mode to Multi-Party for a consultation. Single Speaker is for dictating on your own after the patient has left.
